This issue occurs when the LB connection reuses the port quickly.
This issue is resolved in VMware NSX-T Data Center 3.2, available at Broadcom downloads.

Workaround
Edit the HTTP profile used by the virtual server and disable the NTLM/Server keep-alive flag.
