Please complete this form with as accurate detail as possible. If the form is missing any important information for your specific environment, please let Support know.
Note: If you plan to upgrade more than one environment, such as Development, QA, or Production, please fill out a separate form for each environment.
Once you have completed the form, open a Severity: Low - 4 Support case, asking support to review the upgrade plan with you. After Support completes the upgrade review, the Support case will be closed. If you experience any issues during your upgrade, please open a new support case, detailing the problem.
