Fail to save search filter in user_query table

After applying the SP04 in SDM 14.1, customer is not able to save a search filter in user_query table

03/12 07:38:01.40 CASDMAPPSRV2   web:local             280 ERROR        freeaccess.spl       33468 In add_saved_search, error creating saved searches parent folder: Bad arguments.  Does not match signature.
03/12 07:38:51.41 CASDMAPPSRV2   web:local             280 ERROR        parse.c               3951 Bad data type for arg args

I believe above errors are causing the issue. Just with the stdlogs we cant narrowdown the issue. 
I see it is trying to add a row to user_query table (SQL Name: usq) and somehow it is failign to add.



For some reason, the SP04 installation was corrupted or missing any step
