Unable to start the start request form. The process associated with this form is either deleted or does not exist.

This error can occur if the process path in SRF is incorrect. To resolve this error, login to Process Automation, double click on the SRF from the Library Tab. Click on Properties tab and correct the path within the "Process Path" field as shown in the below example screenshot:
Click on Save. Now relogin to ServiceDesk and should be able to attach the IT Process Automation Workflow to the change category.
