U00015012 error, agent cannot be reconnected

Issue/Introduction

Agent stopped working. Restart of the agent doesn't solve the problem.

Error messages:

20180314/214452.241 - U00015012 'AUTOMICQA01_1' is still in cache. Agent will be disconnected after waiting for PWP. 
20180314/214452.241 - U00011650 Server 'UC4_QA#WP002' / Client '0000': Host 'AUTOMICQA01_1' ended abnormally. (Index='0000000018' CP='MQ1CP001')

Sometimes this can overload the PWP and cause an outage.

Cause

Defect - The agent was unable to be cleaned from PWP cache hence being denied for reconnection

Resolution

Fix: The issue was addressed as a bug and hot fix 12.1.1hf1 was delivered to fix the issue.

Workaround: a COLD start could temporary fix the issue.
