Root Cause: The errors on the universe.log indicate that the launches data file (u_fmsb60) cannot be accessed and/or that the index data file is corrupted.

Resolution

In order to fix the issue, simple stop the node and launch an offline reorganization:

1. Load $U environment
2. Go to the bin folder: cd bin
3. Stop the node: unistop
4. Launch the reorganization offline: unireorg
5. Start the node again, launches should be created correctly now.
