Fail to save search filter in user_query table

After applying the SP04 in SDM 14.1, customer is not able to save a search filter in user_query table

I believe above errors are causing the issue. Just with the stdlogs we cant narrowdown the issue. I see it is trying to add a row to user_query table (SQL Name: usq) and somehow it is failign to add.
For some reason, the SP04 installation was corrupted or missing any step

customer reapplied the SP04 and the problem was solved
