When the status to a CA SDM ticket is updated, there are multiple updates created within the ticket's Activity Log. Any comment added to the USER DESCRIPTION field is the fourth (4th) Activity Log update.
Therefore, to include the comments within a CA SDM notification use: @{act_log_all.3.description}
