A "Failed to initialize" error occurs when attempting to retrieve the status of a PAM UA (Utility Appliance) group.
The error is typically caused by stale authentication credentials or a synchronization failure between the PAM appliance and the Utility Appliance group services.
The Utility Orchestrator logs, available on page (Configuration/Diagnostics/Diagnostic Logs/Download), shows an error similar as the following:

Steps taken to resolve this issue:
1. Rebooted PAM UA v2 Utility appliance. If the reboot doesn't help, remove all Utility appliance devices from PAM and redeploy all to UA device group.
2. Follow instructions as per page Reset the Utility Appliance Password to reset the password.
After, all PAM UA services will display with the correct status.
