Access and Completion

2. Edit: Use Adobe Acrobat Reader to fill out the document.
3. Submission: Once the questionnaire is complete, perform one of the following actions to ensure Broadcom is aware of your upgrade status:
   • Reach out to your Account Director or Customer Success Manager.
   • Open a Support Case to request the creation of a Hot Site for project awareness and attach the completed document to the support case.
