Multi-level approval fail to create step 2 reviewer

Issue/Introduction

Configure custom workflows that step 1manager review and any revokes in this step will create a new task for step2 or application owner. 
However, a few revokes did not create a task for step 2.  This is an intermittent issue 

Environment

Identity Governance 14.5 Standalone with custom workflow process

Cause

Auto-revoke expired campaigns enables the behavior 

Resolution

1) Disable Auto-revoke after campaigns expire from IG. See the doc for the configuration

2) Create a custom kettle script to auto deny step 1
