Cannot move or delete columns and fields

Issue/Introduction

I'm designing a form for an offering.  At first, I could add, modify, move, and delete fields with no problems.  In one section of the form I added a column layout with two columns, and added fields to both columns.  Still everything was working as it should.  But, after moving all the fields from the second column to the first, and deleting the second column, then adding the second column again, I could no longer make any udpates to the form.  Could not add, modify, move, or delete fields and columns.  How can I fix this problem?

Environment

Service Catalog v17.4.x

Cause

In querying the usm_form_entities tables, I found that the form_entity_id field for one of the items in my form had an invalid value.  We suspect this could be causing the problem.
