Issue/Introduction

Symptom:

Adapter rescan takes a lot of time or errors out with the message: "An error occurred while communicating with the remote host"

In the logs, you may find the below information:

Cause

Continuous READ (0x28) errors on device(s) caused path flaps, heavy I/O retries, and latency caused the Storage rescan to fail.

Resolution

• Investigate and remediate the recurring READ (0x28) command failures and the underlying storage path degradation that is leading to sustained I/O latency and connectivity disruptions across multiple LUNs.

• Please engage the storage vendor to perform a detailed analysis of the affected devices and validate the health and performance of the storage subsystem and associated network paths.
