Confirm driver/firmware combination of the storage adapter in use is supported as per Broadcom Compatibility guide
To identify any dead storage paths within the environment, either the esxcfg-mpath -b command or the esxcli storage core path list command can be utilized to display the active/dead status of all connected storage devices
