Issue/Introduction

Fiber channel storage is not visible to the host when doing a storage scan
Fabric switch and see the WWN of the host and zoning is configured correctly

Environment

VMware vSphere ESXi 

Cause

Based off the Broadcom Compatibility Guide the HBA adapter on the host is not on a supported driver and/or firmware


In vmkernel logs you may see the following type of messages:

vmkwarning: cpu14:2097903)WARNING: lpfc: lpfc_map_topology:10509: lpfc1  2019 FW does not support persistent topology Using driver parameter defined value [Loop then P2P]

Wa(180) vmkwarning: cpu32:2098338)WARNING: ql_fcoe:vmhba69:FipDiscoveryTimeoutWork:126: Max retry exhausted for Fabric = 0x############ vlan_id = 0x###

Resolution

Confirm via the Broadcom Compatibility Guide that the Fiber channel HBA is on the correct Driver and Matching firmware 

If issue continues after driver and firmware are updated get the physical adapter checked and engage your fabric vendor to investigate and mitigate the issue.

Once the problem in the fabric layer is mitigated, the impacted hosts may need a reboot to refresh the information on the adapters.
