Issue/Introduction

Symptoms:

Unable to add the disk to disk group post replacing the defective capacity disk.

Validation: 

  • Disk group creation failed with the following error:

A general system error occurred: AIO zero write failure to device /vmfs/devices/disks/naa.################ exception AIO write failure for device naa.################

  • Device statistics indicate that there have been numerous failed read/write operations.

  • Command: esxcli storage core device stats get

Environment

Cause

  • The replacement disk that was added to the system was also identified as faulty, as it failed to initialize correctly and exhibited the same issues as the originally defective disk.

Resolution

  • Engage the server hardware vendor to replace the defective drive and ensure the replacement disk is verified to be healthy and compatible, allowing successful detection and mounting for system consumption.
