Issue/Introduction

Bad vSAN disk on Slot 3 controller 4 was evacuated and Disk Group was unhealthy

Environment

VMware vSAN OSA (All Versions)

Cause

vSAN Capacity disk failed on one of the vSAN Host.

Resolution

1. Contact Hardware vendor to get a replacement disk.
2. Put host into Maintenance Mode with ensure accessibility option 
3. Remove the disk from the disk group and add replacement disk back into the disk group by using steps in the following tech doc
