Cause

An unrecoverable medium error during read/write operations on a physical disk typically triggers this issue causing VM guest OS corruption.

• The /var/log/vmkernel.log file contains entries indicating an unrecoverable medium error encountered on a physical disk:

Resolution

Engage your server hardware vendor to identify the affected physical disk reporting unrecoverable medium error and take appropriate corrective action
