The Upgrade Precheck shows an error "ESXI host connection could not be established", and in the Password Management tab of the SDDC Manager UI, the svc-vcf service account for that host shows a status of "disconnected"

If the host has been rebuilt/reconfigured and then re-added to the vCenter without going through the decommission / commission process from the SDDC Manager, the service account may be missing on the ESXi host.
Follow the steps in this KB to address the missing svc-vcf service account account.
