Issue/Introduction

In the live vmkernel logging accessed via iLO/iDRAC etc (Alt - F12):

<YYYY-MM-DD> <Time> cpu23:2256964)J6: ####: Addr3_ResolvePhys failed for txn 0x############ vol <DATASTORE_NAME> with Address temporarily unmapped

Environment

VMware ESXi 7.0.x

VMware ESXi 8.0.x

Resolution

To resolve the issue, consider restarting the ESXi host or unpresenting the problematic LUN from the storage array.

After the errors cease in the live vmkernel logging, perform a rescan on the host that had access to the LUN.

Engage the storage vendor to investigate the cause of the failure.
