SLM Portlet shows empty spaces for Notes, Excluded Period, and/or Calculation Settings

Issue/Introduction

In the SLM portlet, when creating a new SLA or viewing an existing one, if we click on the dropdowns for Notes, Excluded Periods, or Calculation Settings we see empty rows.

The empty rows show "Required" as if we are supposed to enter something there and we are unable to save or update the SLA without entering a value.
