Issue/Introduction

A drive has failed in the storage array.  The array is showing a degraded volume.  After the drive is replaced and fully reconstructed, the drive continues to show as failed.  

Environment

Dell ME4
Dell ME4084
Dell VA084

Cause

Stripe consistency on the replaced disk is good, so that does not point to a disk issue. There is a communication issue on PHY 13 and per the PHY maps it points to either the SAS cable or the IOM on the A path (Controller A). 

Resolution

Check all physical connections to make sure the SAS cables themselves are firmly attached and locked into their ports.  If the drive has been replaced and the connections look good, then the controller will need to be replaced.  Contact technical support for assistance in opening an RMA with Dell to replace the IOM (controller module).
