• In esxi /var/run/log/vmkernel.log, there are error messages below
  ScsiDeviceIO: 4124: Cmd(0x45b99a8f06c8) 0x8a, CmdSN 0x3f0 from world 176285395 to dev "naa.####" failed H:0x8 D:0x0 P:0x0

Cause

This status is returned when the HBA driver aborts I/O. It also occurs if the HBA resets the target.

Resolution

Engage the storage and hba vendor.
