PSOD occurs due to a possible race condition between removal of a target port with outstanding IO or Report LUN command.
Device driver and firmware may not be compatible as per Broadcom compatibility matrix Broadcom Compatibility Guide
Kindly engage the Hardware vendor (Cisco) to further investigate the issue.
It is advised to update the firmware as recommended by Broadcom Compatibility Guide along with driver. Make sure the device driver and firmware for both the NFNIC and NENIC are both upgraded as per Hardware vendor recommendations.
