Issue/Introduction

  • One or more Datastores are inaccessible
  • All virtual machines associated with the affected datastore are showing as Invalid or Orphaned.
  • vCenter may also be down if it resides on the affected datastore.
  • On the ESXi host, you see PERM LOSS messages similar to any of the following:

[from /var/log/vmkernel.log]

    • vmkernel: cpu#:2097648)VMW_SATP_ALUA: satp_alua_issueCommandOnPath:1019: Path "vmhba##:C#:T#:L#" (PERM LOSS) command 0xa3 failed with status Device is permanently unavailable. H:0x0 D:0x2 P:0x0 Valid sense data: 0x5 0x25 0x0.
    • vmkernel: cpu72:2098604)StorageDevice: 10643: Device naa.############################## APD Notify PERM LOSS; token num:1
    • vmkernel: cpu2:2097394)ScsiDeviceIO: 4689: Cmd(0x457a02946000) 0x##, CmdSN 0x2104 from world 0 to dev "naa.############################## " failed H:0x1 D:0x0 P:0x0

[from /var/run/log/hostd.log]

    • Hostd[2099261]: -->          value = "naa.##############################"
Hostd[2100061]: [Originator@6876 sub=Vimsvc.ha-eventmgr] Event 338 : Lost connectivity to storage device naa.##############################. Path vmhba##:C#:T#:L# is down. Affected datastores: Unknown.

Environment

  • VMware vSphere ESXi 7.x
  • VMware vSphere ESXi 8.x

Cause

  • A Permanent Device Loss (PDL) condition has occurred. The ESXi host received a SCSI sense code from the storage array indicating the device is no longer available or the LUN has been unmapped/deleted on the array side.

Resolution

1. Verify the LUN presentation on the storage array.
2. Check physical cabling and fabric switches for hardware faults.
3. Contact the storage vendor to investigate NO_CONNECT errors and PERM LOSS status.
