An alarm already have ticket got created successfully by the policy.
Later on, it seems like action is being triggered again to create a new ticket for the same alarm (and getting failure) either by policy or manually. In this case, it should have not occurred as there is already a ticket created for the alarm. Lifecycle events show the initial ticket creations as success and second try which occurred many hours later as failure.
Issue was on updates sending to ITMS and the form being sent was for dummy so it was invalid, thus error message back from the ITMS.
