Storage Paths missing for iSCSI based LUNs on Pure FlashArray after ESXi update

Issue/Introduction

Symptoms: 

• You are using Pure FlashArray SAN using ESXi Software iSCSI Adapter
• After upgrading ESXi host, some storage paths are missing (related to one particular SAN controller)
• We see CHAP authentication errors in vobd.log on the affected ESXi host similar to the following:

2025-03-10T16:46:24.765Z In(14) vobd[2097956]:  [iscsiCorrelator] 260657405587us: [vob.iscsi.target.login.error] vmhba64 @ vmk2 failed to login to iqn.XXXX-YY.com.purestorage:flasharray.
abcdef0123456789 because the target returned a login status of 0201

Environment

• VMware vSphere ESXi 7.x
• VMware vSphere ESXi 8.x
• Pure Flash Array
• iSCSI

Cause

• CHAP authentication credentials no longer valid on one controller after upgrading with dynamic discovery enabled
• Dynamic Discovery is not supported when using CHAP with Pure FlashArray

Resolution

• Remove all existing static and dynamic targets from Software iSCSI Adapter Configuration on affected ESXi host(s): Configure Dynamic or Static Discovery for iSCSI and iSER on ESXi Host
• Remove CHAP (do not just update passwords) in FlashArray GUI for the affected Host  (FlashArray GUI ->  Storage Tab -> Choose Host -> Details -> [gear icon])
• Re-add CHAP configuration in FlashArray GUI (FlashArray GUI ->  Storage Tab -> Choose Host -> Details -> [gear icon])
• Add iSCSI static targets (only) to Software ISCSI Adapter on affected ESXi host(s) in vSphere Client with appropriate CHAP authentication information (see link above, as well as, Configuring CHAP Parameters for iSCSI or iSER Storage Adapters on ESXi Host)
• Rescan Storage on Host (Right-click on the host in the Inventory View -> Storage > Rescan Storage):  Performing a rescan of the storage on an ESXi host
