Cause

This status is returned when a device is reset due to a Storage Initiator Error. This typically occurs due to an outdated HBA firmware or possibly (though rarely) as the result of a bad HBA.

Resolution

Confirm you are on the correct driver/firmware version with hardware vendor. If you are on the correct version of the driver/firmware then you will need to investigate the HBA hardware.
