The ESXi host lost connectivity to the local storage controller

Issue/Introduction

• The following alert will appear on the ESXi summary page within the vSphere client:
  
  
  • Lost connectivity to the device t##.NVMe____###_########_Gen##_Boot_Controller______############### backing the boot filesystem /vmfs/devices/disks/t##.NVMe____###_#########_Gen##_Boot_Controller______################. As a result, host configuration changes will not be saved to persistent storage 
    
    
• The following messages will appear in the /var/run/log/vmkernel.log on the ESXi host:
  

  YYYY-MM-DDTHH-MM-SS-###Z cpu##: #######)ALERT: smartpqi01: pqisrc_heartbeat_timer_handler:####: controller is offline with heartbeat value #x##############

• For ESXi hosts running on HPE hardware, the below log messages will appear in the iLO Console:
  
  
  • The iLO health monitoring status of the device / adapter located in the Embedded SAS controller ## is not responsive.

Cause

This issue occurs when the ESXi storage stack loses communication with the Local Storage Controller.

Resolution

Engage the hardware vendor to investigate the issues with the local storage controller.
