This issue may occur due to cache information in LCM in SDDC Manager.
To resolve the issue, restart the SDDC Manager Services.
Run the following command in SDDC Manager Appliance.
/opt/vmware/vcf/operationsmanager/scripts/cli/sddcmanager_restart_services.sh
Another resolution for this is to run the host upgrades in serial mode instead of parallel.
