Issue/Introduction
Symptoms:
ESXi 5.5/6.0 hosts with Qlogic (qlnativeFC) storage driver experiences PSOD.
In the PSOD, you see stack entries similar to:

Resolution
This is a known issue affecting ESXi 5.5/6.0.
This issue is resolved in qlnativefc Driver version 1.1.77.1-1 ( ESX5.5 ) and 2.1.50.1-1 (ESX 6.x) , available at VMware Downloads ( ESXi 5.5, ESXi 6.x).
