Symptoms:
With specific multi-domain environment, after user finished RSA or Radius auth step, the AD auth may always failed, although the AD username and password is correct.

Environment
VMware vSphere 5.x
Cause
HTML Access do not allow hide domain option to be selected at the same time the option enforce 2-factor and windows user name matching is selected.
Resolution
To resolve this issue, unselect either enforce 2-factor and windows user name matching or hide domain in Connection server admin page.
