NSX Manager fails to boot up with error "UNEXPECTED INCONSISTENCY; RUN fsck MANUALLY."

Issue/Introduction

NSX manager may report inconsistency in filesystem and fail to boot up. This has been observed post issues with storage infrastructure.
The following errors may be observed while rebooting manager node:
 

/dev/sda3: UNEXPECTED INCONSISTENCY; RUN fsck MANUALLY.
fsck exited with status code 4
The root filesystem on /dev/sda3 requires a manual fsck
Rebooting automatically due to panic= boot argument

Additional Symptoms Reported:
  • NSX-MGR showing storage errors. Attempted reboot per alarm steps, and manager is no longer responsive.
  • Looks like invalid login attempts are also filling the screen when viewing through vSphere.
