Error: "Failed to read from file: vmware-vcsa" while installing or upgrading to VCSA 6.0

Issue/Introduction

• Cannot install or upgrade to VMware vCenter server appliance 6.0.
• Installing or upgrading to VMware vCenter Server Appliance 6.0 fails with the error:
  
  Failed to read from file: vmware-vcsa
   
• This error is reported after accepting the certificate warning.

Environment

VMware vCenter Server Appliance 6.0.x

Cause

This issue occurs when the installation is initiated from an extracted folder.

Resolution

Mount the ISO and then run the installer.
