Installing ESXi/ESX host on HP Proliant BL servers fails with the error: lvmdriver failed to load

Issue/Introduction

Symptoms:

• Installing an ESXiESX host on HP Proliant BL servers fails.
• When accepting the licensing agreement, you see the error:

lvmdriver failed to load

• On the Service Console of the ESXi/ESX host, you see an error similar to:
  
  vmkctl.hostctlexception: unable to load module /usr/lib/vmware/vmkmod/vmfs3: failure
  
  
• You see this issue on these servers:
  
  
  • HP BL 685c G7
  • HP BL 680c G7
  • HP BL 495c G5
  • HP BL 465c G7
  • HP BL 460c G7
  • HP BL 490c G7
  • HP DL 380p G8
  • HP BL 460c G8
  • HP DL 360 G8
