Symptoms:
16G and 32G Qlogic FC HBAs with PCI device IDs 0x2071 and 0x2261 are not detected after upgrading to ESXi 6.0 U3.

Environment
VMware vSphere ESXi 6.0
Cause
This issue occurs because new HBA device IDs are not available in the inbox driver.
Resolution
This is a known issue affecting VMware ESXi 6.0.x. where in this version, there currently no resolution.
- To workaround this issue, uninstall the 2.1.50.0 inbox version of Qlogic driver and install the latest async driver.
