Full page crash when viewing App page Services tab

Issue/Introduction

When viewing the Apps page Services tab, you may see a full page error message. The error is:

TypeError: Cannot read property 'service_plan_guid' of undefined

Cause

When Apps Manager tries to show a deleted Service Instance that was previously bound to the App, it will fail.

Resolution

When attempting to show a bound Service Instance that has been deleted on the App page Services tab, we no longer attempt to show the Service Instance.

Workaround:

Refresh the page.
