If you are looking to change the default text in the form and remove/replace any text that are not required, you can follow these steps:
Service Desk Manager: 17.x
1) Take a backup of the two files do_svy.htmpl form and view_svy.htmpl files
2) Search for the text you wanted to remove or replace, for example, "Highlighted questions (if any) require a response before submitting the survey " is a default line in the survey template and remove this from the htmpl files.
3) Save the files and copy the edited files to the site\mods folder under SDM installed folder.
4) Run a pdm_webcache -H.
5) Open the survey template and confirm the change.
