Modern UX Custom Investment Type: Issue & Risk on Create getting Error 'Undefined'

Issue/Introduction

When creating new Issues and Risk using the Create Dialog option only, We are getting an Error Message popup 'Undefined'.

Steps to Reproduce: 

1. Create custom attribute (in this case date field)

2. Place that attribute under Risk/Issue Create modal in the MUX->Administration->Blueprints

3. Enable Create Dialog for Risk/Issue modules in the Blueprint

4. Deactivate the attribute created in step 1 in the Classic->Studio.

5. In MUX set Blueprint modified in Step 3 to the project you will be opening to create risk or issues.

6. Go to MUX and attempt to create Risk/Issue using New Risk/Issue button.

7. Notice "undefined" banner error.

Expected Results: Banner should let user know which attribute it is having issue with or once attribute is deactivated it should be removed from Blueprint just like when attribute is deleted.

Actual Results: It is throwing an error and user can't understand what is causing an issue.
