Can not create maintenance window

Issue/Introduction

Unable to create maintenance window in OI. I get some error with JavaScript and page is blank. I can see first screen of maintenance window process, but when I selected agents etc. and click to next, the next page is blank.

Release : 21.3

Resolution

Found out from Engineering that this was already found and fixed and will be released in the next rollout.  This was released 12/16.
