In certain Identity Manager environments, Policy XPress (PX) policies used for account orchestration—such as assigning or revoking Provisioning Roles—may exhibit inconsistent status reporting. While a child event within the task correctly shows a "Fail" or "Error" status, the parent task incorrectly finishes with a status of "Completed."

This issue occurs when the Response settings in the Advanced tab of the Policy XPress configuration have been modified from their default values.
If the response logic is not configured to propagate failures, the parent task will not recognize the child event's failure as a reason to mark the entire task as failed.
To ensure that the parent task correctly reflects the failure of internal Policy XPress events, restore the Advanced settings to their default values:
