To facilitate a successful transition to a currently supported version, the Support team provides a structured checklist for planning your environment move. This document ensures all technical prerequisites are met before the upgrade begins.

If you are planning an upgrade, please utilize the questionnaire attached to this article.  Following these steps helps the support organization maintain awareness of your project timeline.

Access and Completion

    1. Download: Click the PDF file attached to this article to download the questionnaire.
    2. Edit: Use Adobe Acrobat Reader to fill out the document electronically.
    3. Submission: Once the questionnaire is complete, perform one of the following actions to ensure Broadcom is aware of your upgrade status:
      • Reach out to your Account Director or Customer Success Manager.
      • Open a Support Case to request the creation of a Hot Site for project awareness.
