"Unable to connect to the IM Provisioning server"

Attempt to log into the provisioning server via an LDAP browser such as Jxplorer and confirm that your credentials are in fact correct.

If this is working is having one issue with the Workstation where Provisioning Manager is running.
Try to install again the 14.4 VirtualApplianceTools.exe to install Provisioning Manager. Provisioning Manager setup is done during the installation and no more required for your to run the Setup program. Once the installation is complete just run the Provisioning Manager and you should be able to connect to the Provisioning Server.
This can also be caused by different version of IMPM and rest of the system
