The Entity ID is not correctly set up in Clarity - MUX - Administration - SAML Configs
It should match the IDP and be something like this: https://<servername>:<port>/niku/nu#action:union.samlMetadata
This issue can arise during database refresh as well. In newer versions the Entity ID is built using the SAML Config ID as /niku/sso/<SAML_CONFIG_ID> at the time of import. Need to change this manually after the import to match that of the IDP.
