In Service Point multi site offering fields are not displayed

Issue/Introduction

Create a service with multiple sites in the catalog, login to the service point and fill in the fields and click on the submit button. After that when we look at the services we no longer can see the fields only buttons are visible.

Steps:

1. Login to Service Point URL
2. Create a Request ticket with the multiple site offering
3. Submit the request
4. Once submitted - Click on my tickets to review the request 
5. Note the offering form object pages disappear and only the buttons are visible

Resolution

This was handled via defect - DE61381 and the fix is included in 17.3 RU13.
