Seeing this error during installation/upgrade process:

code:28,000, AUTHORIZATION_FAILURE

Cause

Permissions not set due to a mistake during the the replication setup process. 

Resolution

Rerun through the entire replication setup process to ensure that the documentation is followed properly and the values entered are correct.
