When assigning an Access role in CA Identity Manager (IDM) it is possible to make comments compulsory (see below).
Is it possible to make comments compulsory in the CA Identity Portal?

There is no mandatory setting for comments in the portal. However, you can meet your requirements by having a new tab on the permissions form, then bind the comments to a string and set it as mandatory. Example below:
When you come to add the permission, the extra tab is displayed for you to enter comments.
If you try to proceed without adding the comments, the validation fails.
The additional comments can be seen in the approval screen by expanding the box.
