No data was populated in the securlet activity or investigate.
Resolution
Disable Enforce login IP ranges on every request under SalesForce Setup | Security Controls | Session Settings
It is recommended to relax IP restrictions during activation and then add IP restrictions back.
The In the Login IP range is listed in the SalesForce Securlet Techdoc.
The Current range:
