Portal is sending 2 request to the same approval after submission

Issue/Introduction

vApp 14.2 with SQL Server 2012 as DB, experiencing a problem while submitting a request:

Instead of sending a request, it sends 2 requests to the same approval. And the approval needs to rejects or approve both requests for it to proceed:

This happened after restarting vApp services. Changing workflow doesn't work, nor importing a new workflow. Here's our workflow config:
