Alarm Analytics page should be hostname or hostname and agent name

Issue/Introduction

The entity name for APM alarms in Alarm Analytics page should be either hostname or hostname and agent name. As a result, users can quickly glance through the hostnames easily.
Currently, the entity name is too long and it is not easily read until you hover over each of them or expand the column wide.
